Athletic PE Exemption Form
- For 10th-12th grade students only.
- Athletes must be on the roster the entire season of one of our CIF Sports listed below.
- Students enrolled in any PE class are not concurrently eligible for a PE exemption.
- It's the student's responsibility to fulfill the team requirements {as designated by the coach} and to turn in paperwork by the deadline.
- It's coach's responsibility to notify Athletics that the student fulfilled the season with the team at the conclusion of the season.
- The fall deadline is August 28, 2025 - Cross Country, Field Hockey,
Flag Football (G), Football, Golf (G), Tennis (G), Volleyball (G), and Water Polo (B). - The winter deadline is December 3, 2025 - Basketball (Boys, Girls), Cheer (Competition), Soccer (Boys, Girls), Water Polo (G), and Wrestling (Boys, Girls).
- The spring deadline is March 17, 2026 - Baseball, Cheer (STUNT), Golf (B), Gymnastics, Lacrosse (Boys, Girls), Beach Volleyball (G), Softball, Swim & Dive, Tennis (B), Track & Field, and Volleyball (B).
Complete this form once you have been named to a team's roster, and turn it into Athletics office by the deadline noted. Drop off the forms school days (Monday – Friday, between 8 a.m.-3:45 p.m.) {no appointment necessary}. *There will not be drop-off availability during non-student days/District breaks/holidays.*
Alternate Athletic Packet
We strongly encourage using HomeCampus.com. If you select the "alternate athletic packet" option, your clearance and/or tryout could be delayed. If you have registered at Home Campus, an "alternate athletic packet" is not required.

Alternate Physical Form - the Medical Eligibility form (page 4) must be completed in its entirety and turned into the Athletics office
NCAA Information
Students are responsible for notifying their counselor of their intent to participate in college athletics. It's important to reach out to your counselor during your Freshman year if you plan on attending a D1 or D2 school, to ensure you are taking the proper classes and completing the needed requirements for admission.
